One of the most common questions Irish parents ask is whether their child can get braces for free through the HSE. The answer is yes — but only for a minority of children with the most severe problems. Here is how the system actually works.
Is orthodontic treatment free in Ireland?
The HSE provides free orthodontic treatment, but it is reserved for children with the most serious bite and alignment problems. Most children do not meet the threshold, and adults are not eligible at all. The medical card does not cover orthodontics.
Who qualifies for HSE treatment?
Eligibility is decided using an internationally recognised grading system, the Index of Orthodontic Treatment Need (IOTN). The HSE funds treatment only for children who fall within grade 4 or 5 — the most severe categories. Qualifying problems include very prominent front teeth, buried or missing teeth, or significant issues with the bite or jaw development. Around four in five children do not meet this standard.
How does referral work?
Children are usually screened through the HSE dental service, often via a school dental examination around 6th class. If a severe problem is identified, the dentist refers the child to the local HSE orthodontic unit for assessment. Importantly, referral must happen before the age of 16.
What about waiting lists?
Because resources are limited, HSE orthodontic care is prioritised by severity, and waiting times vary by region — often stretching to a number of years between assessment and the start of treatment. The most urgent cases are seen first.
What if your child does not qualify?
If your child does not meet the grade 4 or 5 threshold — or you would rather not wait — private treatment is available and can begin at the most beneficial time for your child's development. Private treatment also opens up discreet options such as clear aligners and tooth-coloured braces. Remember that you can claim 20% tax relief on private orthodontic costs through a Med 2 form, and most clinics offer interest-free payment plans.
The bottom line
